What should I do as a follow up to this risk assessment?

I went to a property to carry out a repair to a customer immersion heater and during inspection and testing, found the tails were in single cores insulated in green and yellow (both neutral and the line) with blue/ brown tape to differentiate the tails in a metallic enclosure without a sheath for mechanical protection.

After my risk assessment, I found a potential danger whereby any damage in the single insulated wire causing the wire to touch the metal casing. Therefore i need your best advice...?
